The Macmillan Personalised Care Team focus on meeting the health, wellbeing and rehabilitation needs of cancer patients diagnosed or treated at Great Western Hospital (GWH).
The team can refer to multidisciplinary services within GWH including psychology and dietetics, and will liaise with Clinical Nurse Specialists to support prehabilitation and rehabilitation.
