The Heart Function Service provides support and advice to you and your family following an admission to hospital with acute or decompensated chronic heart failure.

We review the control of inpatients' symptoms and ensure that you are offered appropriate investigations and treatments.

We provide advice about medication and education, about how to manage your heart condition when you go home.
